I can confirm there's no memory on this board. There's not even a microcontroller for this, nor does this device have any memory.
I've never heard of a form like this. Does it need to be filled out by someone within Texas Instruments?

I do have a form for this that I can email to a TI representative. This has become common with companies that want to protect proprietary information that may inadvertently be saved to a purchased device. Usually if there is memory in a device, especially non-volatile memory, a procedure is provided by the manufacturer to clear this memory. In this case, the form will serve as proof that there is no memory in the device, so it is not a concern for proprietary company information.
