OPA348: Device Failure

Dear  Team

We installed 100pcs of this device and found that the output was not functioning properly.

When we input 0V into the oscilloscope, the output should also be 0V, but the actual output reached 0.4V.

We conducted an ABA verification and found that the abnormal device was causing the issue.

Any suggestion on what I have mistaken or missed?

    Were all 100 non functioning?

    In the output waveform the output is bimodal, what changes in the setup? (power on , off perhaps). What are the power supply voltages?

    Try adding a 10pF or bigger capacitor in parallel with the feedback resistor to stabilize the feedback loop.
