We use OPA4141 in one of our products. We're seeing a fairly high failure rate of close to 1% of units per year coming back with catastrophic failure of the op amp, with visible burning of the package. Are there any known reliability issues with this part? Indications are that the failure occurs spontaneously just after power on.

I am not aware of known quality issues in OPA4141 recently. There are recent PCN issued, but it should not affect the failure rate or quality issues.

Could you send me the OPA4141 schematic that is related to the mentioned failure?
The type of failures you mentions are often related to EIPD (External Induced Physical Damage) that potentially exceeding the abs. max. ratings.
I would like to take a look at the circuit and perhaps we are able to find root causes.
