The tube carrier option is inefficient and was removed as an option. The R simply means "reel"

The LM139AD orderable part number is obsolete. The tube option is no longer available.
The LM139AD will only be available in the 2.5k reel, as Ron pointed out, the "R" stands for reel. So the LM139ADR lives on.
However, since the LM139AD orderable is discontinued, most likely the devices on the shelves will be the "classic" die.
The LM139ADR could be either the "classic" die or the new TiB die, as mentioned in the appnote, as it is in production and covered by the PCN.
