THS4131 Amp NOT WORKING CORRECTLY

The data sheet specifies a max differential input of plus and minus 6 volts.  I need to know if this applies to the single ended configuration at gain of one in the figure on page 18, figure 38 of the data sheet.  That is exactly how I'm using it.  I am powering the device with a bi-polar plus and minus 12 volt supply.

The part works great with an AC signal of 10V peak-to-peak down to about 10Hz.  As the frequency goes below this the -Vout output begins to drop.  At DC the -Vout output stays stuck at zero volts.

According to the datasheet the part should work with static DC levels.

I want to give the part an input signal of up to plus and minus 10 volts, DC to 100KHz, powering the THS4131 using a + - 15V supply.  Is this a problem in this configuration?  Why is the part not functioning correctly down to DC?!

I contacted tech support last thursday and I still have not received any answers.  Any help is appreciated before I'm forced to move on from a TI solution.
