AMC1301: No output issue

Hi team,

My customer is using AMC1301 for 3-phase motor current sensing. Issue is that there is no signal output on the output end.

They use 5V power supply on primary side and secondary side. Shunt resistor is 10ohm.

Issue rate is 4/170. They use 3x devices for 3-phase and issue will appear on different positions.

Issue moment, they can measure the 5V on both sides.

Could you please help review the schematic and do you have insight on this case?

    This schematic doesn't have values for the components. 

    Please provide a full schematic and layout. 

    For the devices that have failed, please provide detailed probing location and voltages through the schematic signal chain.
