The test conditions for the entire specification table is listed at the top of the table. Thus the conditions for the OPA189 test are: T = 25C, Vs = ±18V, Vcm = Vout = Vs/2 = 0V., and RLoad = 10k and is connected to Vs/2 (0V in this case). The test conditions listed next to the specification are exceptions or in addition to the test conditions at the top of the table. In this case the condition Av = 1 is in addition to the conditions at the top of the table. Thus, a buffer configuration is used and the input sinusoidal signal is swept until the output drops to -3dB or 0.707V/V.

About test signal , Do we have frequency ,amplitude or other requirement ?
As customer use 50mV signal and +/- 15V power to test OPA189 GBW, test result is around 9M, So I want to know the different.

The signal should be less that 100mV to avoid slew limitations. The frequency is swept until the -3dB attenuation is achieved. That is you look for the frequency where the gain drops by 0.707 V/V
