As discussed on the chat today I want to start a Failure Assessment for a
TI Op Amp
that was purchased from Mouser by the Brisbane manufacturer Circuit
Solutions. Theo Mitrakis the Engineering Director of Circuit Solutions is
CC'd in this conversation.
This chip is used in a commercial product that is in early stage
production. Several chips have failed on several PCBs, sometimes the
replacement part also failed and was replaced.
The function of the opamp in the circuit is a simple unity gain buffer for
an ADC input with bipolar supply of +/-9V.
The failure observed is that the non-inverting input is drawing current
loading up the approx 1.5V signal from the previous peak hold stage. See
attached schematic.
We understand that the TLV171 is superseded by the TLV9351. However we are
not convinced that simply replacing the opamp with a newer model is going
to resolve the issue unless we can understand the root cause of the
problem.

In addition to the email below, I realised that the schematic snippet
shows U15 already updated to the newer part TLV9351. This is to clarify
that the failures happened with the TLV171 in current production boards.
