OPA828: OPA828 get damaged

Hi,

I’m working on a TIA design using the OPA828 op-amp.
 
The TIA input is connected to an axial wire soldered to a metal pin, which I then enclosed with heat-shrink tubing. I used a lighter to shrink the tubing.
 
After doing this, I found the OPA828 had failed.
 
Could this have been caused by ESD generated by the lighter? I don’t see any direct ESD path to the metal pin, and the device includes internal ESD protection diodes.
