THS4561: NOT GETTING PROPER SIMULATION RESULTS

I am simulating single to differential conversion using THS4561 in ti tina as per application note. But not getting proper output.4561.docx

    You don't have a Rg on the non-driven side. You need to match the 1kohm R3 on the undriven side like so. 

    Your gain is also 16V/V how you have it, the 2Vpp input you are simulating is too large and the output will clip. Also the resistor divider between the supplies on the undriven side is going to set a DC voltage which will also be gained up as a difference between that divider and the DC value of the source. Not sure what the point of that divider is.
