The nominal spacing between adjacent pins of the INA146 is 0.030". A commonly used estimate is that voltage breakdown in dry air is 50V per 0.001" spacing. So an estimate for breakdown between pins would be 1500V in dry air.
Of course, conditions are unlikely to be always dry air and this does not allow for possible contamination or other possible smaller breakdown paths on a circuit board. These conditions are beyond our knowledge or control so it's difficult for us to provide definite answers. Under common operating conditions, however, it is generally not difficult to support the voltages 100V levels specified for the INA146.
Some pretty obvious cautions apply. Avoid liquid contamination. Avoid condensation. Avoid a buildup of dust and dirt.
