Is there a single supply rail-to-rail In/Out op amp whose input range includes ground and whose output will swing to within at least a few mV of ground like the OPA340 does but has at least a 12 V supply capability ?

Low speed is fine.

Take a look at the OPA703/4/5, TLV237x family or TLV2781. The OPA703 CMV is (V-)-0.3V to (V+)+0.3V The output is within 40mV or the rails typ with a 100kohm load (this varies depending obviously on the load. [:)])

Other potential options include the OPA734/35 or a member of the TLV237x family.

Application is an adjustable voltage controlled current source :

But since the MOSFET would not conduct if its gate voltage is say that 40 mV of the OPA703, that device should be usable for this. I am using a low gate threshold logic level MOSFET (RFP30N06LE with a 1 V gate threshold). Does gate threshold vary with drain current ?

I would check the datasheet for the specific characteristics of the FET you are using.
