INA826 linearization output is not expected

INA826 inputs +62.5mV,but outputs only +1.9V. It should be 64.5mV*(1+49.4/1.2)=2.719V. Can anybody tell me why? What problems does my circuit happen?

Hi Pete Semig,

I just use the dual supply for the INA826 like -5V and +5V. Maybe the single supply for the INA826 causes lower INPUT COMMON-MODE VOLTAGE and OUTPUT VOLTAGE.
