THS6214 Iadj Value and polarity not clear

For operating a THS6214 between +/-12V power and for maximum bandwidth,  should Iadj be connected to ground, +12V or -12V and what is the resistor value? I assume the Bias would be set to Full for the maximum bandwidth. The circuit is that of figure 81 of the data sheet.
