OPA129 High Resistance measurement



I am using the OPA129 as a high resistance measurement opamp at Rin. My schematic is attached.

This is the buffer configuration as shown in the datasheet Figure 2B.

The power supply is an Acopian DB15-10. V+ is +15V and V- to -15V.

Initially, the circuit worked effectively. It has worked for weeks. However, for some unknown reason, it abruptly stopped working. I am seeing lower output voltages than expected. I replaced the device and it started working again. However, it stopped working a second time!!

Do I need some sort of circuit protection? If so, how do I protect the circuit?
