Help with removing the offset

Hi, I am using the UAF42 to create a butterworth filter in the noninverting configuration. My resistor values are RG = 50 ohms, RQ = 1 M ohms, RF2 = 474 kilo ohms, RF2 = 148 ohms. I am powered it with +/- 9V i have my input going into pin #2  but I do not have pin 12 connected to anything and my RQ is connected to pin 3 and to ground. For some reason when i put in a 100mV sine wave with a frequency of 500Hz I get a signal that has a -6V offset. The weird thing is the  the filter did not do this before and now I don't know why I have this large offset. Is pin 12 supposed to be connected to something? 

Please help as this is very urgent. Thanks
