LMV341: IC fails and draws a high current on the supply rail

We use an LMV341IDCKR op amp as part of our design for voltage sensing. The uniploar supply voltage is 5V. The input voltage can be expected to vary between 0 and 80V and there is a resistor dividet to step it down. We have sever of these modules(1000s) running in the field and have recently had a unit returned in which this particular IC failed. Is there any particular reason that could cause this failure?

    I do not see any problems with the circuitry. In what way does the amplifier fail? Can you provide the voltage on all pins during the failure?

    Does the part overheat or pull down the supply rail due to the high supply current?

    Does this fail in production or a field return/ Do you know the failure rate?
