LM2904: LM2904 failure issue

Dear all:

     We used LM2904 for voltage-follower circuit, attached is design circuit.

    

     Input voltage 0-10V   Vcc= 11.7V.   We have 15 pcs damage(PIN7 voltage=VCC=11.5V,and not change).  Please help to check circuit design? we need remove design issue,  thanks!
