OPA2340: determination of date/batch code format

We have experienced an increased failure rate recently with a couple of different batches of this product.  We'd like to understand what the batch code format translates to in case this is being caused by product from a particular fab.  Could you advise what format we should expect and how it breaks down?

    TI cannot disclose details about the OPA2340 marking because of the problems with semiconductor product counterfeiters. Revealing our confidential marking code information only helps counterfeiters undermine our efforts to put an end to their practices.

    You didn't provide any information about the increased OPA2340 failure rate. What has the failure rate been, and what has increased to? What failed electrical characteristics do the failures exhibit? If you provide the schematic of the OPA2340 application circuit with supply, input signal, and output load and any environmental conditions, we can review them with regard to the product reliability issue.

    If there is a valid reliability issue with the OPA2340 product you received, TI has procedures in place with our authorized TI distributors to address such issues.
