OPA192: OPA192 output issue

Hi,

    One of our customer use OPA192 as attached circuit. the input is 3V but the first OPA192 output is about 2.89V. We checked the schematic but can't dig out the issue.

   Pls help double check and advice what's wrong?

    From the looks of it, I suspect that the output of OPA192 is unstable (osciallates) or it's damaged. Do they use bypass capacitors of at least 0.1uF on both supplies to ground?  Please place a scope probe on the output of each amplifier and determine whether they are stable.  In order to help your customer further, we need a full schematic with no floating nodes - see below.

    Also, it is critcial to know the size of the cap and the type of diodes used  - see below.
