make stand alone buffer

Hi 

I ordered some LMH6321MR/NOPB buffer amps, i attached the picture of it. I have two questions here, hope some technicians can help me figure them out:

1. I have problem to identify each pin on the chip, i do not know which one is V+, which is Vin etc. since i think the manual does not give the diagram clearly.

2. my purpose is to use LMH6321MR/NOPB as a single stand alone op amp, the manual only talks about how to use it in closed loop, can anyone give me a schematic of stand alone based op amp by using LMH6321MR/NOPB.
