What is the signal at the output of TSENSE? Is it voltage, current? How does the output scale with a change in temperature?

TSENSE is just the connection to the anode of a diode which can be used for a die temperature measurement of chip. The cathode of this diode is connected to GND and by the help of an external circuitry you can drive a constant current through this diode while the voltage drop across the diode is a measure of its temperature. It's the classic temperature dependency of the voltage drop across a silicon pn-junction which is used here. Figure 45 of datasheet shows how the TMP411 can be used for the temperature measurement, e.g..
