On dual +/-15V supplies, INA149 can support the input common-mode voltage range up to +/-275 with respect to supply ground. Thus, it does not matter how you call the current shunt side voltage (ground, high-side, or low-side) BUT the only thing that you must make sure of is that the voltage on either side of the shunt does NOT exceed +/-275V relative to INA149 supply ground. Even though the input common-mode voltage may be up to +/-275V, because of INA149 fixed gain of 1, the differential input voltage (voltage across the shunt resistor) must be betweeen -13.5V and +13.5V - see below.
