Main requirements:

1. Voffset = 3 mV MAX

2. Ib = 260nA MAX

3. Slew Rate = 2.2V/µs and above

4. Gain = 3Mhz and above

5 Vcc=3.3V

These are high cost devices.
ADI cost $0.40/20KU
Another competition is ON SEMI, p/n NCS2003ASN.
Need SOT23-5
package

Against the AD8691, take a look at our TLV376. This device meets / competes many of the parameters you outlined. The TLV376:

Against the NCS2003, can you specify the requirements? I can suggest the OPA320.
