OPA454: Unable to sense the input

Hi,

My client encountered an issues with OPA454 as it could not sense the input from the range of 0-40V. Can I have a schematic review?

    We can certainly review the schematic. Please be sure to provide the complete schematic and information about the power supplies being applied, the input drive conditions and the output load conditions.
