Hi Shadders,

I think you have short-circuited R3 somewhow when experimenting with the circuit. This would not only explain the damaged OPA192 but also the damaged zener diode.

By the way, I don't understand, why you have mounted back to back zeners at the -input of OPA192. They cannot protect the OPA192 at all.
