have you seen the "recommended operation conditions" in the datasheet?
A minimum supply voltage of +/-3.5V corresponds to a minimum single supply voltage of +7V. So, your single supply voltage of +5V is too low but the +12V, on the other hand, is acceptable.
Keep in mind, though, that the TLE2064 is no rail-to-rail OPAmp. So, it has a very limited common mode input voltage range and the output voltage cannot swing to the rails. Have a look into the datasheet please. ("Rail" is here another word for supply voltage.)

I agree with Kai. The part will not be able to handle the single, +5V supply.
To further clarify Kai's second comment, a quick jog through the datasheet will show the low end of the input common mode voltage is about 3V above the negative rail. So, this part may not be desirable for your +15V application either. Although, it will depend on your needs for the input and output voltage ranges.
