HELP: ACF2101 Offset Error Too Large

I have implemented the programmable voltage to current converter that appears in Fig 10 of the spec sheet.  In the output, however, I am not using SELECT (but the unswitched output) and I am going to a current feedback based inverter with a gain of 1.  My problem is that the HOLD and ~RESET pulses are coupling noise to the output.  More precisely, if I connect the integrator Sw In to ground, every time the HOLD pulse is issued a mirror of this pulse appears at the output going slightly above -1V.  This offset due to coupling seems excessive when compared with the spec sheet.

 

Below I am inserting my actual circuit and the output description.  I am implementing the circuit on a proto board with discrete 30AWG wires; wiring does not seem to have an effect on the output just by playing with it.

 

Any suggestions ASAP would be greatly appreciated!!
