There's a lot of information missing here to be able to answer that question.
Right off the bat, an issue that I see that is most likely causing problems is capacitor C16. The customer has placed no isolation between the output and the capacitor, and the INA199 is only capable of driving a maximum 1nF load:
The capacitor shown is 100x the allowable maximum and is certainly causing stability issues on the node.
If this doesn't alleviate the issue, we need to make sure that the error is coming from the INA199, and not further up the signal chain. Could I get a corresponding input plot to see what this output corresponds to? Also, are the inputs being driven with a sine wave? What is the output expected?
