I am using TINA-TI for the first time and it seems like a very useful tool. However, I tried putting together a simple integrator circuit with a single supply op amp. When I input a 5HZ square wave, I get a 5HZ square wave at VF2 as shown in screen shot below. This same circuit is referenced in "Op Amps for Everyone" by Mancini (Fig A-32).
Any ideas what I might be doing wrong?

I don't think that you did anything wrong. That is how the circuit supposes to behave at low frequency (e.g. 5 Hz). The -3dB break point is at 1.59kHz. BTW, the author missed the s parameter in the transfer function.
Enclosed is the simulation. At 5Hz, Vout = -Vin. Try higher frequency above the break point at 1.59kHz. The circuit is not pure integrator (remove R2 and recalculate R1 and C1 to convert to a pure integrator with Cdv/dt = i).
