A problem in implementing OPA657

Hello everyone, I am really green in using op-amp. and recently i was asked to implement one circuit using opa657. i found even i use it as a simple adder, there is quite large noise (about 4V sometimes) when i scale to 5us/DIV in the oscilloscope. it seems i didn't use the opa657 correctly. can anyone give me some idea how to solve the noise problem?

    Can you please attach a schematic of the OPA657 circuit you are using and an oscilloscope capture with the noise you are seeing? Are you seeing this noise with no input signals? The OPA657 could be oscillating - the minimum recommended gain is +7V/V.
