From some reason I get no answer from EVB on the TDO line (using TI GUI )
the SDO line is "stack" at 1.2V and never response , When attach a scope probe the responses of all registers is 0x00 and without a probe return 0xFF at all registers
it happened on 2 EVB (out of the box)

1. attach a photo of 2nd EVB ( at the 1st one I had change to 5V operation by shorting RB- and adding bias to input)
2. see PD jumper as small influence (the right one is the -2.5) first image low power
GUI at 2 option (with probe connection and with out)

we had replace the IC's and both boards are now working !!! ( maybe we had some damage when make the touch-up)
2nd issue drive from single end source
at simulation when working in single source (drive INP with 0.7V on Common mode ) I get at output, as expected, two signals with 180 phase
but on EVB , the outputs are at same phase
where is the problem ?
note: we use the amplifier in 5V single power and the common mode is set to 2.5V

Glad to hear you were able to get the parts to work! Note in regards to your other issue, the LMH6401 is designed to work only with differential inputs. See image from the datasheet below:
