LM48511: Damaged after a day long of use

To Whom This May Concern:

I am evaluating the LM48511 audio amplifier in a prototype, and initially, it seems to be working well.  I've since run it all day (2K sine wave input) generated from an audio codec (NXP SGTL5000), and some time in the afternoon it has become a soft tone.  After powering several other units (5 others) they all seem to have been damaged and are no longer outputting any tones.  I think I've clearly have a design issue, and I was hoping to get some help resolving my issue.  I will be happy to send my schematic and any associated layouts to help with the issue.  I am also wondering if I have encountered a thermal issue (no heat sink or pads; only ground plane to standoff for heat transfer).  I am wondering if I need to add a heatsink or tune down the gain to reduce the heat generation.
