TAS5634: Incorrect Output from Amplifier IC

I am requesting assistance from TI regarding my issue with the TAS5634 IC. I have a know good reference configuration from the TAS5634EVM which is producing the correct output and I have dumped that configuration onto our prototype board. What I am having trouble with is that the output of the amplifier IC looks like constant noise instead of a sinusoidal output. The PWM modulator (TAS5548) has been flashed correctly and is producing the required PWM signals to feed into INPUT_A, INPUT_B, INPUT_C, and INPUT_D accordingly for 2x BTL configuration.

When I capture FFT data from the output of the prototype board I cannot see the carrier frequency harmonic or any test signal that I am injecting. Therefore, I believe there is either a misconfiguration, board layout issue, or firmware configuration issue that I am not seeing.

I am more than willing to talk support and show current schematics/gerber files for good engineering design practices to point me into the right direction.
