TPA6211A1-Q1: Short circuit of PIN5 and PIN7 occurred on several products

Hi E2E,

Our customers have found short circuits between PIN5 and PIN7 on multiple products in their applications.

The SCH as followed:

The power supply is provided by MPQ4423HGQ-AECQ, and the loudspeaker is KPB3642SP1-8222.

Are there any circuits on the schematic that need to be corrected?

    The schematic for TPA6211A1 itself looks OK.
    The speaker specs also seem OK for this amplifier.

    Is customer running tests on the supply side of the circuit? Make sure the regulation is proper across different test cases. Would also check the PCB layout, how VO+ is routed to the speaker, is there any possible short to GND or higher voltage rail? Please notice the device is able to protect against short to voltage up to VDD, higher voltage than VDD may cause damage to the device.
