TPA3004D2: Volume Control shorts to ground after few power on offs

Hi TI,

We have a design to control the 8 ohm speaker using the TPA3004D2. The design schematic is also attached for reference. 

When we have a new board, it works fine but when we repeat the tests, after few power cycles, we observe that the Volume control pin shorts to ground. The volume control is done using DAC output from a microcontroller. Can you please look in to the design and suggest what might be going wrong with the design. We replace the IC, it works fine and again after few power cycles, it goes bad. Looking for best possible solution.
