LMK05028EVM: Issue locking with 10Mhz square waveform input using LMK05028EVM.

I received LMK05028EVM board and tried evaluate LMK05028.  I am not able to lock.

My system will need the following:

1. 10Mhz square waveform input at IN0

2. 125Mhz LVCMOS output at OUT0_P

3. 128Mhz LVCMOS output at PUT1_P

However,  when I input 10Mhz and press S3, all D8, D9, D12, and D13 are turned on.  It is indicated that DPLL1, and PPLL2 are not locked.

It seems like EEPROM is set up to input 25Mhz clock.

By the way, I used 25Mhz input to lock 125Mhz on OUT0.  I am not able to lock 128Mhz on OUT1 some reason.
