Is it necessary to configure all registers upon startup? Or is it sufficient to configure only those registers that need to be changed? (In other words, is there no need to reconfigure registers that remain at their default values?)

    Only registers that need to be changed need to be written, with one exception: 0x168 (LSBs of PLL2 N divider) must be written to trigger internal VCO calibration if internal VCO is used. As noted in the datasheet, 0x171, 0x172, 0x17C, and 0x17D must be written before writing 0x166, 0x167, and 0x168. Note that it is only 0x168 that triggers calibration, so technically 0x166 and 0x167 may be written at any time beforehand.
