With VDD = 3.5V, You cannot configure internal reference as 2.048V, since we have x2 output buffer which will cause clipping of the output stage.
Max Capacitance allowed on Ref pin is 100pF in internal reference mode.

With max value capped at 3V, it may be OK. You need to verify the same as we didn't test the device for the same conditions. Most likely it will be OK ( Please verify).
As far as CAP on ref pin is concerned, 10uF Cap can affect in multiple ways, first and foremost being stability of the reference circuit as it can oscillate, again please verify. Secondly your start up and slew time can be impacted. Damage to the device not likely

The broken IC has no REF and OUT outputs.
Is it due to the REF capacitor or VDD?

Whats the current on the VDD pin in the broken part?
Can you try to operate all the devices in the normal operating conditions, as anything violating datasheet spec can have issues with device.

The current of VDD after startup is 0.89mA, and 1.2mA after setting the internal register.
There seems to be no short inside, do you know anything?

When you are operating device under recommended conditions did the device fail?
If yes, we may need to do FA on the same to identify root cause for the same. How many devices failed? whats the failure rate?
If no, I request you to stick to device recommended operating conditions.

Reference is set to external by default. Need to double check with my designer as this is very old part and datasheet info is very limited.
