DLPA200: DLPA200 voltages not present

I have designed a custom PCB using the DLP system (DLPC410, DLPA200, DLPR410, DLP7000).  In my design, the DLPA200 is not providing the VBIAS, VRESET or VOFFSET voltages that are expected--all are 0V.  The V5REG rail is at 5V, RESET is high.  Can you point me in a direction of things to start looking at?  Are these voltages supposed to be present at all times once the chip is powered?  What could keep the chip from providing these voltages?  I have provided a picture of the schematic as well.
