TUSB2036: Problems with device connection

Hello!

I'm having some problems with the hub. 

I'm developing a device with 3 ports downstream, to a linux device. The system recognizes the hub as a 3 ports hub. The problems starts now:

- The system only recognizes plugged devices in port sequence 0, 1 and 2. If I connect to usb port 1 or 2 before 0, It doesn't recognizes the device. The following picture ilustrates whats the system shows.

- The second problem I'm having is when you put a USB device in port 0, it recognizes and the device works good. But when you remove the USB device the HUB doesn't detect and in the system it appears as connected but without any device.

The schematics of my devlopment is attached bellow. 

These problems could be happening by the hardware configuration?

    Do you have the 15K pulldowns / caps / series resistors on the downstream ports?  Assuming that those are included, this appears to be a software issue where the USB host is not reading the status from the hub on disconnect / reconnect.  Has this driver been confirmed to work with USB Full Speed hubs?
