Has the test been repeated across multiple systems?  I recently debugged a similar issue, and it turned out the USB cable was not fully connected.  I would recommend changing out the hardware to narrow down root cause.

we have tested in multiple systems. i would say 2 out of 10 system is having this issue.

Based on the current description of the issue, I would say it's unlikely the root cause is the TUSB3410.

Have you been able to switch out the hardware (motherboard, cable, device, etc)?  You can swap the parts between the 2 non-working systems with the 8 other passing systems to narrow down the physical location of this problem.  The purpose is to see if the issue follows the motherboard or the cable, or something else.  Then we can focus on debugging that component.
