AM26LS31: Failure mode in which the output is reversed

Hi,

A failure mode has occurred where the output is reversed due to a failure in AM26LS31. Is it possible to get some opinions on possible reasons for the failure, since I don't mind using the internal block diagram in the datasheet?

  • Is this permanent?

    I can only assume that this is caused by exceeding the absolute maximum ratings, usually overvoltage at some pin(s). Is it possible for overvoltage to reach the input or supply pins? How are the outputs protected?

    So if this is permanent it is a problem and indicates some kind of fault with the device which could have various causes. 

    As Clemens also mentioned, this could possibly be explained by electrical overstress - is there any application of signals above what is stated in the datasheet and do you have protection on the line, if so what does it look like? I understand that you may not be able to post a schematic snippet of how the device is connected - but if you can it would be very helpful to look into from my side.

    Also could you take a picture of the top of the suspect unit showing the part markings? - I can use that to trace it back into our system to ensure it is a legitimate device.
