TPD3S014: The TPD3S014DBVR can burn (under certain unknown conditions) on my design

Some of our users have reported a chip burn with the TPD3S014DBVR.

They are using USB 2.0 devices, connected on the HOST port of our PCB.

Our design is supplying 5V (1A) on the host port, protected by the TPD3S014.

I received the products back for the maintenance, and indeed, I can see that the TPD3S014 is burn, and not providing the +5V output anymore (the +5V input, supplied internally on our design by a regulator, is still working).

Unfortunately, I can't reproduce the issue, it's rare but happen with some users. 

Any ideas of how this issue can happen? For example, do you think an input supply voltage >5.5V can damage the TPD3S014?

    THe ABS MAX table shows the voltages, currents, temperatures that are likely to damage the part.

    For USB systems, the most common failures is an over voltage condition where VBUS is driven over 6V for long enough to damage the device.

    It is also possible for repeated ESD events to damage the device, but this is unlikely given the 12KV/15KV IEC esd rating of this device.

    I would focus your debug on determining if VBUS or any other pin was driven above 6V.

    Thanks a lot for helping me.

    I've checked the VBUS and it seems it can't go upper +5V

    I've checked with my users and they all get the issue with the same USB controller. Maybe this USB controller is consuming too much current and damage the IC ? Or maybe it's the ESD? I will look further into it.

    If their device pulls a large amount of surge current on power up, then it is possible to damage a protection device during power up.

    Do you think an external circuit can help to improve this behavior and protect the TPD3S014 ? Do you maybe have some ideas of circuits I can add?
