So... what is needed to terminate an unused non-removable USB output outside of the USB spec if OTP, I2C, SMBus are not used?

    Sorry, my initial response was not clear.  There is never any need to physically terminate an unused or non-removable port.  The USB specification does require a hub to report a non-removable port or an unused port, but functionally the hub does not operate differently based on these configurations.
