I am using this level translator for MDC/MDIO , reset, interrupt connections.
Pull up to be placed on either sides (pins 3,4,5,6?)
Pull up value can be 4.7K ?

Pullups are recommended on both the inputs and outputs of IC42. For IC76, pullups are needed on B2, but optional on A1/A2/B1 if both RESET and INT signals are push-pull driver based.
The value of the pullups are selected based on the desired speed on the signals. For estimation, this equation can be used:

For IC42, adding 4.7K pull up as recommended by the receiver device.
Also, same pullup value is to be used on either side as it is a PASS device?

This is not a firm design requirement for the device, though note that during a logic LOW, the pullups on the I/Os will become parallel.
