SN74LVC1G139: Driving leds causes high temperature and unstability

We have designed a battery module using a BQ-25171-Q1 and need to separate stat1 and stat2 to an RGB LED, so we used an SN74LVC1G139 to decode from 2 bits to 3 outputs.

The logic gate changes based on the status from BQ-25171-Q1, but it gets hot (approximately 100 °c), and sometimes, both outputs are active simultaneously.

We've tested on three different PCBs, and the problem exists on all of them. We've attempted resoldering the SN74LVC1G139 and aren't able to locate the fault. See the attached PDF for schematics.

Battery voltage is nominally 2.4 (2 NiMH in series).

I hope you can advise. Since our problem exists on multiple boards, we assume it must be a design flaw on our end.
