Hi Cannie,
It looks like you're translating from 2.5V to 3.3V. This is right on the edge of the voltage requirements for the LSF -- we typically recommend having at least 1V between the two supplies, but 0.8V is ok.

I would recommend switching to the single supply bias circuit, which is shown in the EVM's user guide: www.ti.com/.../slvub53.pdf
