The OSC resistor is NOT within the acceptable range given in the datasheet. In the datasheet (page 11), the ROSC range is given to be 160kΩ<ROS<2MΩ. 

Please increase the resistance and check if issue persists or not.

The PWM frequency that the customer needs to output is 40KHz. According to calculations, ROSC needs to be set to 100K Ω.

This resistor should only be used to set the frequency, right? Why does it affect the output of KEEP pins?

I think the calculator tool only does the calculation based on the equation given in the datasheet for ROSC. But it does not take into account the limits for ROSC (160kΩ<ROSC<2MΩ). ROSC values below 160kΩ may be interpreted as default OSC settings (OSC connected to GND). Most likely what is happening in your situation is that the OSC is bouncing between default and 40kHz causing the misbehavior.

Can you replace the OSC resistor and test if the issues disappears?
