My customer start up synchronous rectifier burning MOS DS Pin . This kind of failure phenomenon has about 4 sets, a total batch 500 sets.

Could you give any suggestion how we locate where the problem is ?

Could you please give more details description and share more information. such as:
1. What the primary side controller part number.
2. what the input voltage and output voltage /load when failure happened. looks like there is PFC at front stage , what is bulk voltage ?
3. What the transformer Lm . turns ratio , and what the SR FET P/N
4. Did you captured any waveforms when the failure happens , it should including primary FET Vds , SR FET Vds and Vgs , It's more better have transformer primary winding current.
